#34 Add an 'enrollment options' option

Provide a way to let a user who requests a certificate provide us with command line arguments which we will provide for use by an enrollment helper, possibly in an environment variable which we'll expand if we see it in the command we'll execute, as we're preparing to invoke that helper.

New milestone was created for milestone-less tickets.
